ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide supervision and direction to assigned residential homes.  
  • Assist as needed in interviewing/ hiring process for new employees following agency policy.
  • Provide training with new Direct Care Staff to include ITSP, ISP, Therapy Plans, Adaptive Equipment use and maintenance, mock fire drills, med observation check offs for newly trained staff. Turn in training documentation in accordance to training processes.
  • Must maintain training as designated trainer on all plans in homes the Residential Case Manager is responsible for, assure all staff are trained on all plans, and proof of training is in the home and office.
  • Complete and document coaching sessions with direct care staff. 
  • Escalate performance issues to Program Supervisors, including documentation from Coaching sessions.
  • Responsible for maintaining weekly/monthly schedules of staff working in assigned residential homes. 
  • Ensure adequate staffing is maintained in the home to meet all contractual agreements.
  • Responsible for monitoring OT, and Optimal hours. Coordinate with the PS, and RC all schedules.
  • Complete daily documentation reviews.  Including but not limited to:  learning logs, MAR’s, shift change, daily/monthly health over site documentation, weekly summary, and other documents as assigned. Complete weekly RCM reports for each person supported on caseload and to be turned into the Program Supervisors weekly.
  • Complete weekly schedule and coordinate personal time keeping with HR for accuracy in the UltiPro System.
  • Ensure home books are in order, and up to date
  • Responsible for ensuring all appropriate documentation forms are in the home.
  • Provide Training and Mentoring to ensure direct care staff can provide quality person-centered services.
  • Review and submit learning logs weekly for timely billing.
  • Follow agency management plan
  • Transport and support person with medical appointments.  Assure documentation of the visit is returned to the Medical Department the day of the appointment.
  • Work closely with the Medical Department to follow all doctor’s orders and address medical needs quickly and to resolution.
  • Coordinate with Financial Department to ensure shopping, spend downs, and all financial needs of the person are met.  Including turning in, reconciling all receipts in a timely manner.  Ensure check requests are submitted timely on behalf of the person
  • Maintain good working relationships with families, conservators, ISC’s and other entities involved in the person’s life. 
  • Understand and always follow Incident Management guidelines.
  • Meet with Program Supervisor, financial department, HR and medical department weekly for mentoring and training. Also meet with IMC.
  • Perform site inspections and unannounced supervision visits as assigned.  Ensure follow up is completed within 1 week of identifying concerns in the home.
  • Report and follow up on Maintenance issues
  • Promote and assist with community inclusion activities
  • Provide support to fellow RCM staff, and promote a positive work environment.
  • Other duties as assigned.

 ADDITIONAL DUTIES AND RESPONSIBILITIES

  • Follow agency Quality improvement plan
  • Facilitate, promote and insist on person centered practices.
  • Always advocate for the person supported first.
  • Must maintain a punctual and good attendance record.
  • Any other duties as may be assigned.

 PHYSICAL REQUIREMENTS

  •  Physically able to perform the essential functions of the job as described in the essential duties and responsibilities sections including; walking, bending, stooping, kneeling, squatting, sitting, reaching overhead and lifting up to fifty (50) pounds.
  • Must be able to safely lift, transport, and transfer consumers, which may include but not be limited to lifting and transferring in and out of a bathtub, vehicle, bed or wheelchair, as appropriate and needed to assist with the consumer’s assisted daily living skills. 

QUALIFICATIONS

  • High school Diploma or GED
  • One year of experience working with individuals with intellectual/developmental disabilities, or in a related field, preferred.
  • Must be at least 18 years of age.
