Job Description

  • Orchestrate all facets of the active treatment effort, including the IDT creation of relevant IPPs tailored to meet individual person’s needs
  • Effectively coordinate internal and external program services and supports to facilitate the acquisition of client skills and adaptive behaviors
  • Promote competent interactions of residential staff with clients in program implementation and behavior management
  • Routinely observe individuals across settings in program areas to assess effectiveness of program implementation and consistency of training effort to determine effectiveness of IPPs and making timely modifications to facilitate achieving desired skills or goals.
  • Routinely interacting with program staff across settings to assist in determining the effectiveness and continued relevance of program plans in meeting identified client needs
  • Determining the need for program revision based on client performance
  • Identifying inconsistencies in training approaches or programs not being implemented as written and facilitating the resolution of these inconsistencies
  • Assures follow up occurs for any recommendation for services, equipment or programs so that needed services and supplies are provided in a timely manner to meet the client’s needs
  • Create annual IPP’s or revisions as needed for each individual in conjunction with information from other paraprofessionals
  • Advocate for individuals rights and protection from harm; identify need for Human Rights Committee reviews when needed, collect necessary consents and information for review
  • Assess the needs of the person receiving services through completion of periodic assessments such as the following: ICAP/MRRC, Comprehensive Functional Assessments, Vocational Assessments, Restraint Risk Assessment, Suicide Assessment, and Legal Assessment.
  • Act as an advocate and quality assurance monitor to ensure services are being provided in the most efficient and effective manner.
  • Secure and maintain client’s eligibility funding in accordance with specific program regulations.
  • Maintain ongoing communication with local, state and federal entities.
  • Perform all job duties in adherence to state and federal laws concerning confidentiality, including HIPAA Privacy Laws.
  • Facilitate ongoing training and skills development for DSP’s and nurses working in the home.
  • Interact with a wide variety of professionals including but not limited to: physicians, social workers, psychologists, private employers, school administration, therapists, law enforcement, and various other members of the community.
  • Conduct periodic chart audits.
  • Complete and facilitate Admission, 30-day, Annual and Discharge Staffings.
  • Complete and facilitate all IDT meetings and IPP revisions.
  • Complete quarterly progress report for assigned consumers.
  • Complete enrollments and transfers, and all corresponding paperwork by due date(s).
  • Complete assessment, paperwork and facilitating meetings and services for temporary and permanent discharges.
  • Conduct and document monthly contact with assigned consumers and conservators.
  • Conduct routine home inspections to report any concerns including related to health and safety as well compliance with DIDD & CMS rules and regulations.
  • Assist with Legal status including guardianship and advocacy needs.
  • Monitor trust-fund requests and receipts.
  • Update and monitor consumer information (Back Office Web)
  • Monitor and adjust Levels of Need.
  • Implement behavior plans and monitor for effectiveness.
  • Review and follow-up on incident reports.
  • Correspond with Local Authorities (LA)
  • Monitor all medical services.
  • Identify Abuse/Neglect/Exploitation and follow proper procedures for reporting.
  • Act as the primary liaison with DIDD for surveys.
  • Complete Plans of Corrections for DIDD surveys.
  • Assist with Administrative tasks as needed, such as payroll, staffing, etc

Participate  in on call rotation
